What happened

Delfin Offshore Pipeline's 42-inch gas line failed and the released gas ignited during recommissioning work in Cameron Parish. PHMSA's preliminary findings said a cleaning pig appeared to have hit a closed valve; Delfin later estimated that 56 million cubic feet of gas escaped. The line, which had been out of service since 2012, was depressurized and remained shut in under a federal corrective-action order. Because it was not carrying commercial flows before the test, this record does not treat the failure as a new production shutdown. The investigation remained open and the marker is approximate.
