What happened
A temporary work platform collapsed inside the concrete tower of the F8 turbine while six workers were spreading bedding mortar and preparing to install the next tower section at the under-construction Hunan Xinhua Qiaocheng Phase I wind farm. Five workers, aged 29 to 47, died and one was injured. The project comprised 24 Goldwind GWH204/6.25 MW turbines with 150 MW total planned capacity. Investigators found that the platform had been installed with five attachment points instead of the seven required by its design and without required anti-drop bolts, allowing successive connection failures and a complete fall. The official investigation classified the event as a production-safety responsibility accident, described project safety management as chaotic and ineffective, and referred three people for criminal prosecution. The EPC consortium involved Shanghai Electric Power Design Institute and Shanghai Electric Power Installation Second Engineering, companies reported as linked to PowerChina; that corporate linkage is contextual and is not presented as the technical cause.
