What happened

A security guard entered the fenced movement area of an oil-well pumping unit while it was stopped in automated pump-off mode. The unit restarted without an advance warning and its crank and counterweight struck and killed him. Brazil's ANP investigation identified missing warning signs, unsafe positioning, automatic startup without warning, and failures in hazard communication and risk analysis.
