Petrobras P-19 — accidental CO₂-system discharge

What happened

A fixed carbon-dioxide firefighting system was activated incorrectly while four maintenance workers were inside P-19's auxiliary diesel-generator room. Two escaped and two were rescued; one later died. ANP found design, inspection, risk-assessment, warning and training failures. The operating platform stopped all production the next day and subsequently entered decommissioning; the regulator does not present the fatal discharge as the sole reason for permanent decommissioning.
