What happened

Iranian state media said an unidentified Iranian commercial cargo vessel was struck by an unknown projectile in waters between Hengam Island and the Shib Deraz coast of Qeshm Island early on 13 September. Qeshm Governor Hossein Amir Teymouri was reported as saying one member of the ten-person crew was killed and four were wounded; he blamed a 'terrorist enemy', but the projectile and attacker had not been identified. UK Maritime Trade Operations Warning 134-26 separately recorded a projectile strike on a vessel transiting the Strait of Hormuz, first reported at 23:00 UTC on 12 September. Update 001 said a fire subsequently broke out and local authorities were evacuating the crew. Associated Press described the Iranian casualty report and UKMTO warning separately, and no reviewed source conclusively established that they concern the same ship. This record therefore includes the UKMTO update as potentially related context but does not count it as an additional attack or transfer its fire and evacuation details to the Iranian vessel as established facts.
