What happened

During startup of a renewable-diesel hydroprocessing unit, a misaligned valve and blocked burner air supply caused overheating that ruptured a fired-heater tube, releasing renewable diesel and hydrogen that ignited. One employee suffered third-degree burns over most of his body and roughly $350 million in property damage resulted; the unit was shut for nearly a year. This is a renewable-fuels processing incident, distinct from conventional crude refining, and was accidental, not an attack.
