What happened

A feed buffer vessel in the diesel hydrotreating unit overpressurised, exploded and caught fire after high-pressure process material flowed backward through failed or insufficient isolation following a compressor and pump trip. Two people were killed and one lightly injured. China’s Ministry of Emergency Management identified the failed check valve, absence of emergency pump-outlet isolation and delayed manual intervention as key direct factors.
